| Investment Advisor Agreement (PDF) |
This form asserts the advisor agrees to comply with the Gift Fund's Charitable Investment Advisor Program Guidelines (PDF). It is only required for the first Giving Account from your firm. Please contact us to learn whether your firm already has an Agreement on file. It must be signed by someone who is authorized to act on behalf of the entire firm, such as an officer or principal. |
| Benchmark Selection Form (PDF) |
This form allows you to select the benchmark against which your investment decisions will be monitored for a specific Giving Account. It must be completed and signed by the advisor. |
| For the Donor | |
| Gift Fund Donor Application (PDF) |
This application form initiates the Giving Account set-up and must be signed by the donor. This form is only required for new Giving Accounts. If looking to enroll an existing Giving Account in the Charitable Investment Advisor Program, this form is not needed. |
| Donor Enrollment and Advisor Nomination Form (PDF) |
This form allows the donor to nominate their investment advisor. It must be signed by the donor and the nominated advisor. |
| Contribution Form (PDF) |
This form is required if the donor is making new or additional contributions to the Gift Fund. It must be signed by the donor. If looking to enroll assets currently held in an existing Giving Account, this form is not needed. |
